Medicare supplement plan N: what the standardised letter covers

Plan N is a standardised Medigap letter, which means the benefits behind it are fixed in federal law and identical whoever issues the policy. It is usually described as the cheaper alternative to Plan G, and that framing hides what actually separates them: Plan N is not a smaller version of the same thing, it is the same thing with two specific costs handed back to the holder.

Where it differs from the more complete letters

Plan N covers the hospital coinsurance and additional days, the blood benefit, hospice coinsurance, skilled nursing coinsurance, the Part A deductible and foreign travel emergency care. It leaves the Part B deductible to the holder, as Plan G does. What is distinctive is the other two: Plan N pays the Part B coinsurance except for copayments on some office visits and some emergency room visits, and it does not cover Part B excess charges at all.

Why the copayments matter more for some people than others

A copayment on an office visit is trivial for somebody who sees a doctor twice a year and is not trivial for somebody managing several conditions with monthly appointments. That is the whole design: the letter trades a lower monthly cost for a cost that arrives when care does. Nothing about this makes it better or worse than the alternatives, and a site that told you which to choose without knowing how often you see a doctor would be guessing.

Excess charges are the part that varies by where you are

A Part B excess charge arises when a provider does not accept the Medicare-approved amount as full payment and bills the permitted amount above it. Plan G covers those and Plan N does not, so the gap is only as large as the practice of excess billing where a person actually gets care, and several states restrict or prohibit it outright.
